Notice of Security Incident
On April 12, 2017, Dermapathology Laboratory of Central States (“DLCS”) became aware of a security incident related to the information portal used for viewing its pathology reports. Upon learning of the incident, DLCS immediately launched an investigation into the matter. Following the investigation, DLCS determined that between October 6, 2013 and April 12, 2017, a vendor that provides information portal services to DLCS failed to properly configure a temporary folder to prevent unauthorized access to the pathology reports of 69 DLCS patients. The patient health information contained in the pathology reports includes patient names, demographic information, Social Security numbers, and clinical information.
DLCS’ investigation, which included a sophisticated forensic analysis by a third party and internal reviews of both DLCS and vendor systems, found no evidence of the information having been accessed or obtained by any unauthorized individual or to otherwise suggest criminal fraud or identity theft. Nevertheless, out of an abundance of caution, and in compliance with applicable notification laws, DLCS issued personal notice to those 69 patients that may have been affected by this incident.
